Nondiscrimination notice

Who we treat, and what we will do if the language we speak is not the one you speak.

Heart Health Center provides treatment without discrimination as to race, color, religion, sex, national origin, disability, or source of payment. Every patient has the right to exercise these rights without being subjected to discrimination or reprisal.

Interpreters

If you need an interpreter, please let us know and one will be provided for you at no cost. If you have someone who can translate confidential medical and financial information for you, please make arrangements to have them accompany you on the day of your procedure.

Ask when your procedure is scheduled, or call us on (602) 456-2342, ext. 3000. Arranging it in advance is better than arranging it on the day, but tell us either way and we will sort it out.

If you believe you have been discriminated against

Raise it with us through the grievance procedure. You have the right to have a verbal or written grievance investigated and to receive written notification of the actions taken, and you will not be treated differently for raising one.

You do not have to come to us first. You can file directly with the U.S. Department of Health and Human Services Office for Civil Rights at ocrportal.hhs.gov, or with the Arizona Department of Health Services on (602) 364-3030.
